Pediatric and Adolescent Gynecology: What's the Issue? What's the Answer? What's the Big Picture? What's the National Perspective?

      Our 23rd Annual Clinical Meeting in San Antonio, Texas is a most logical forum for adolescent gynecological care being delivered to the next level. From a national perspective, let's take a look at the very recent edition of the American College of Obstetricians and Gynecologists (ACOG) Compendium section reflective of the “Committee on Adolescent Health Care” and identify a distinct parallel with our annual meeting.

      Specifically, let's take into mind, use of intrauterine contraceptive systems in adolescents. Did you know that the levonorgestrel-releasing intrauterine system (IUS) lowers the risk of pelvic inflammatory disease (PID)? The proposed mechanism is cervical mucus thickening and endometrial thinning.

      Clinical performance of a levonorgestrel-releasing intrauterine system and oral contraceptives in young nulliparous women: a comparative study.
      There does not appear to be any adverse effect on future fertility. Of interest, insertion in the nulliparous patient may be facilitated by administration of misoprostol. This appears to soften the cervix.

      The subject of “New and Old Contraceptive Choices in Adolescents” is one concurrent session at our San Antonio meeting and it provided the means of updating all in attendance regarding what's the issue and indeed what's the answer.
