In the article, a corporal-preserving clitoroplasty technique is used for the first
time in an adult woman with congenital adrenal hyperplasia (CAH). On clinical examination
6 years after the surgery there was a “normal” appearing vulva. Questioning the patient
revealed that it did not only look good but also felt good, because sexual stimulation
gave a swelling of the labia without pain and led to an orgasm. Hisham and Zain have
concerns about the preservation of the erectile bodies.

They believe that this could cause pain and embarrassment. Additionally, there is
the possibility of regrowth, which could increase the problems of pain and embarrassment.

- PreviewWe read with interest the article by Tjalma.1 The goals of feminizing genitoplasty surgery are to achieve morphologically and functionally normal external genitalia with satisfactory sexual response. Dr Tjalma should thus be congratulated for the excellent long-term result obtained in the described case. However, we would like to raise some concerns regarding the corporal-preserving clitoroplasty technique as detailed by the author.
